Diabetes Self-Management Program

The Diabetes Self-Management program at Hilton Head Regional Medical Center is an outpatient program designed to teach people how to successfully manage their diabetes.  Knowledge is power.  Knowing the right way to manage diabetes can make all the difference in living a healthy life with diabetes.

Our program provides training on proper nutrition, blood glucose monitoring, physical activity, stress management and preventing diabetic complications.  Motivation and support are provided to allow for positive lifestyle changes.

Professional staff includes a certified diabetes educator, registered nurses, registered dietitians and exercise physiologists.  Together this professional team provides a well rounded approach to helping you control diabetes.

The Diabetes Self-Management Program is recognized by the American Diabetes Association and maintains its national standards.

  • Understanding Diabetes
  • Balanced Nutrition
  • Meal Planning & Carbohydrate Counting
  • Living Healthy with Diabetes
  • Stress Management
  • Physical Activity
  • Insulin Instruction (when needed)
  • Monitoring and Controlling Blood Glucose
  • Preventing/Managing Short & Long-term Complications
  • Local & National Diabetes Resources

Participants must be referred to this program through their physician.  Group and individual sessions are available.
